This statement, which includes a Medical Questionnaire, the Discover Scuba Diving Safe Diving Practices and a Liability Release and Assumption of Risk Agreement, informs you of some potential risks involved in scuba diving and of the conduct required of you during the PADI Discover Scuba Diving program. Your signature is required to participate in the program. If you are under 18 years of age, you must have the Participant Statement (which includes and acknowledges the Medical Questionnaire, the Discover Scuba Diving Safe Practices and the Liability Release and Assumption of Risk Agreement) signed by your parent or guardian.  If, after completing the Medical Questionnaire, you need to have the PADI Medical Statement completed by your physician, we have provided a link to the Medical Statement.

This is a statement in which you are affirming that you have been advised and thoroughly informed of the inherent hazards of scuba diving activities.  You are also providing a liability release and express assumption of risk associated with scuba diving.  Please read carefully, fill in all blanks and initial each paragraph before signing at the bottom.  Your signature on this statement is required.  By signing it, you are also stating that you have been fully informed of the contents of the release and assumption of risk.  If you are under 18 years of age, you must have this statement signed by a parent or guardian.  In all cases, a witness must also sign and date the document.
